Introdução: A comunicação é reconhecida como uma habilidade central por vários órgãos reguladores internacionais da educação médica. O ensino específico de habilidades de comunicação é fundamental para melhorar a comunicação dos médicos. As técnicas experienciais mostraram superioridade em comparação com os modelos tradicionais. A utilização de consultas reais ajuda os estudantes a visualizar melhor as suas competências de entrevista e a refletir sobre elas. Com os avanços da tecnologia, o uso de consultas médicas gravadas em vídeo tornou-se a abordagem padrão para o ensino da comunicação. No entanto, a eficácia dessa técnica depende do envolvimento ativo dos estudantes. As suas contribuições e comentários dos pares sobre a consulta gravada são essenciais para a aprendizagem. Contudo, a perspectiva do estudante sobre a utilidade dessa abordagem educativa recebeu pouca atenção. Objetivos: Compreender a percepção da aprendizagem dos residentes de medicina de família e comunidade resultante da atividade de vídeo feedback na sua formação profissional. Métodos: Estudo exploratório, qualitativo, realizado com residentes do primeiro ano de medicina de família e comunidade de um programa de residência estabelecido em São Paulo, Brasil. Os participantes foram entrevistados após as sessões educativas, que foram analisadas por meio de análise temática reflexiva. Resultados: A autopercepção de sua prática, o aprendizado de habilidades de comunicação e os ganhos afetivos foram identificados pelos participantes como pontos de aprendizado derivados da atividade de vídeo feedback. Além disso, sobre o aprendizado de habilidades específicas de comunicação, eles mencionaram comunicação não-verbal e verbal, conexões entre teoria e prática, estrutura de consulta e oportunidades para cristalizar conhecimentos. Os ganhos afetivos incluíram sentir-se parte de um grupo, melhora da autoestima, superação de inseguranças, percepção de consultas mais efetivas, reforço do gosto pelo trabalho e reconhecer a necessidade de mais aprendizado. Conclusões: Os ganhos de aprendizagem identificados em nosso estudo levaram a uma experiência de humanidade compartilhada, que permite aos participantes serem mais efetivos técnica e afetivamente com seus pacientes. Além disso, identificamos que a atividade educativa de vídeo feedback pode ser utilizada para outros possíveis fins educacionais além do ensino da comunicação.


Introduction: Communication is recognized as a central skill by various international medical education regulatory bodies. Specific teaching on communication skills is important to enhance doctors' communication. Experiential techniques appear to be superior compared to traditional models. Real-life consultation helps trainees visualize their interview skills and reflect on them. Upgraded by technology, the use of video-recorded medical visits became the standard approach for communication teaching. However, the effectiveness pf this technique relies on trainees' active involvement. Their inputs and peer feedback on the recorded consultation are essential to learning. Despite its importance, their perspective on the usefulness of video feedback in medical education has received limited attention. Objective: To understand the perception of learning among general practice trainees as a result of the video feedback activity in their vocational training. Methods: An exploratory, qualitative study, conducted with first-year general practice trainees from an established training program in São Paulo, Brazil. Participants were interviewed after educational session, which were analyzed using reflexive thematic analysis. Results: Self-perception of their practice, communication skills learning, and affective gains were identified by participants as learning points derived from the video feedback activity. Furthermore, for specific communication skills learning, they mentioned nonverbal and verbal communication, theory and practice connections, consultation structure and opportunities for crystallizing knowledge. Affective gains included feeling part of a group, improving self-esteem, overcoming insecurities, perception of more effective consultations, reinforcing fondness for their work, and need for more learning. Conclusions: The learning gains identified in our study led to an experience of common humanity, which allowed participants to be more technically and affectively effective with their patients. Also, we identified that the video feedback educational activity can be used for other possible educational purposes, beyond the teaching of communication.


Introducción: La comunicación es reconocida como una habilidad fundamental por varios organismos reguladores internacionales de educación médica. La enseñanza específica de habilidades de comunicación es importante para mejorar la comunicación de los médicos. Las técnicas experienciales parecen ser superiores a los modelos tradicionales. El uso de consultas reales ayuda a los estudiantes a visualizar y reflexionar mejor sobre sus habilidades de entrevista. Actualizado por la tecnología, el uso de consultas médicas grabadas en video se ha convertido en el enfoque estándar para la enseñanza de la comunicación. Sin embargo, para que la técnica funcione, la participación de los estudiantes es crucial. Sus contribuciones y comentarios de los compañeros sobre la consulta grabada son esenciales para el aprendizaje. Sin embargo, la perspectiva de los estudiantes sobre la utilidad de este enfoque educativo ha recibido poca atención. Objetivos: Comprender la percepción del aprendizaje por parte de los residentes de medicina de familia y comunitaria como resultado de la actividad de vídeo feedback en su formación profesional. Métodos: Estudio cualitativo exploratorio realizado con residentes de primer año de medicina familiar y comunitaria de un programa de residencia establecido en São Paulo, Brasil. Los participantes fueron entrevistados después de una sesión educativa, que fueron analizados mediante análisis temático reflexivo. Resultados: La autopercepción de su práctica, el aprendizaje de habilidades comunicativas y las ganancias afectivas fueron identificadas por los participantes como puntos de aprendizaje derivados de la actividad de vídeo feedback. Además, sobre el aprendizaje de habilidades comunicativas específicas, mencionaron la comunicación verbal y no verbal, las conexiones entre la teoría y la práctica, la estructura de consulta y las oportunidades para cristalizar conocimientos. En cuanto a las ganancias afectivas, relataron sentirse parte de un grupo, mejora de la autoestima, superación de las inseguridades, percepción de consultas más efectivas, refuerzo del gusto por el trabajo y necesidad de más aprendizaje. Conclusión: Los logros de aprendizaje identificados en nuestro estudio llevaron a una experiencia de humanidad compartida, que permite a los participantes ser técnica y afectivamente más efectivos con sus pacientes. Además, identificamos que la actividad educativa de vídeo feedback puede ser utilizada para otros posibles fines educativos, además de la enseñanza de la comunicación.

Resumo: Introdução: Como um componente substancial na relação médico-paciente, a comunicação pode ser determinante na construção da hipótese diagnóstica e na adesão ao tratamento por parte do paciente, e, por isso, há a necessidade de compreender os fatores que influenciam no processo comunicativo e descrever a efetividade dele. Objetivo: Este estudo teve como objetivo avaliar os impactos da comunicação inadequada na relação médico-paciente. Método: Vinte e três pacientes voluntários com a maioridade atingida preencheram questionários de informações socioeconômicas, uso da linguagem e impressões da consulta médica. O médico preencheu um questionário sobre a experiência da consulta. Resultado: Os resultados indicaram que os pacientes apresentaram alguma dificuldade em comunicar ao médico o que sentiam, e, de modo complementar, os médicos, em 20% dos casos, tiveram algum grau de dificuldade de chegar à hipótese diagnóstica a partir do relato do paciente, o que se relaciona com a linguagem pouco descritiva utilizada pela maior parte dos pacientes. Conclusão: Dada a necessidade da qualidade da comunicação entre o médico e o paciente, conhecer os fatores que impactam o processo comunicativo é o primeiro passo para a garantia de um atendimento eficaz com autonomia do paciente e maior adesão ao plano terapêutico.


Abstract: Introduction: As a substantial component of the doctor-patient relationship, communication can be crucial in the construction of a diagnostic hypothesis and patient adherence to treatment. It is therefore necessary to understand the factors that influence the communicative process and describe the effectiveness of the communication. Objective: The aim of this study is to evaluate the impacts of inadequate communication in the doctor-patient relationship. Method: Twenty-three voluntary patients of legal age completed questionnaires on socioeconomic information, language use, and impressions of the medical consultation. The doctor filled out a questionnaire regarding their experience of the consultation. Result: The results indicated that patients experienced some difficulty in communicating their feelings to the doctor, and furthermore, in 20% of cases, doctors encountered some degree of difficulty in reaching a diagnostic hypothesis based on the patient's report, which is related to the predominantly non-descriptive language used by most patients. Conclusion: Given the need for high quality communication between the doctor and the patient, understanding the factors that impact the communicative process is the first step towards ensuring effective care with patient autonomy and improved adherence to the treatment plan.

ABSTRACT

ABSTRACT Medical specialties have recognized that breaking bad news assists clinical practice by mitigating the impact of difficult conversations. This scenario also encourages various studies on breaking bad news in ophthalmology since certain ocular diagnoses can be considered bad news. Thus, the objective is to review the scientific literature on breaking bad news in ophthalmology. The literature databases like MEDLINE/PUBMED, EMBASE, LILACS, SCOPUS, COCHRANE, and SCIELO, were screened for related research publications. Two independent reviewers read all the articles and short-listed the most relevant ones. Seven articles, in the formats of original article, review, editorial, oral communication, and correspondence, were reviewed. Conclusively it reveals that ophthalmologists are concerned with communicating bad news effectively but lack related studies. Nevertheless, there is a growing realization that training in breaking bad news can increase physicians' confidence during communication, thus, benefiting the therapeutic relationship with the patient and his family. Therefore, it would be valuable to include breaking bad news training in the curriculum of residencies.


RESUMO O reconhecimento sobre a comunicação de más notícias como mitigadora de conversas difíceis por outras especialidades médicas, incentiva o estudo desta temática na oftalmologia. Sendo assim, o objetivo deste estudo é revisar a produção de pesquisas científicas sobre a comunicação de más notícias em oftalmologia. Para isso, foi realizada uma revisão de literatura. As bases de dados utilizadas foram MEDLINE/PUBMED, EMBASE, LILACS, SCOPUS, COCHRANE e SCIELO. Dois revisores independentes leram todos os artigos e selecionaram a amostra final. Sete artigos foram escolhidos nos formatos de artigo original, revisão, editorial, comunicação oral e correspondência. Os oftalmologistas estão preocupados em comunicar as más notícias de forma eficaz, mas faltam estudos sobre o tema. No entanto, há uma crescente percepção de que o treinamento de comunicação de más notícias aumenta a confiança dos médicos na comunicação, beneficiando a relação terapêutica. Portanto, seria valioso incluir este treinamento no currículo das residências.

ABSTRACT

Empathy is fundamental to the physician-patient relationship and influences the clinical outcomes. The aim of this study was to determine the dental students' empathy levels and to compare their opinions on attributes of a "good dentist". This cohort research was included 79 dental students volunteering to participate in the study at a public university. The data were collected by the Dökmen's Empathic Tendency Scale (ETS) and Empathic Skill Scale (ESS). In this study, from the orientation day to the end of the third-year, the dental students' ETS mean scores (respectively 69.59 and 68.34) and ESS mean scores (respectively 142.53 and 140.00) showed a decline. At the end of the third-year, starting clinical rotations and taking patient responsibility may lead to decrease the empathy levels of students. The findings of our study show that female students have higher empathy tendency and skill scores compared to male students (p<0.001). In the ranking of the attributes required to be a good dentist, empathy was ranked 4th by the first-year students, while final-years students placed empathy in second rank (p<0.05). According to these results, it is possible to claim that the dentist candidates realized that being empathetic is necessary to be a good dentist.


La empatía es fundamental para la relación médico-paciente e influye en los resultados clínicos. El objetivo de este estudio de cohorte -que incluyó a 79 estudiantes de odontología de una universidad pública que se ofrecieron como voluntarios- fue determinar los niveles de empatía de los estudiantes de odontología y comparar sus opiniones sobre los atributos de un "buen dentista". Los datos se recogieron mediante las Escalas de Tendencia Empática (ETE) y Habilidad Empática (EHE) de Dökmen. Desde el día de orientación hasta el final del tercer año, los puntajes medios ETE de los estudiantes de odontología (69.59 y 68.34, respectivamente) y los puntajes medios de EHE (142.53 y 140.00, respectivamente) mostraron una disminución. Según la literatura, al final del tercer grado, iniciar rotaciones clínicas y asumir la responsabilidad del paciente puede llevar a disminuir los niveles de empatía de los estudiantes. Los hallazgos de nuestro estudio muestran que las estudiantes tienen una mayor tendencia a la empatía y puntajes de habilidad en comparación con los estudiantes (p<0.001). En el ranking de los atributos requeridos para ser un buen dentista, la empatía ocupó el cuarto puesto en estudiantes de primer año, mientras que los de último año colocaron la empatía en segundo lugar (p<0.05). De acuerdo con estos resultados, es posible afirmar que los candidatos a dentistas se dieron cuenta de que ser empáticos es necesario para ser un buen dentista.

O objetivo deste estudo foi determinar os níveis de empatia de estudantes de odontologia e comparar suas opiniões sobre os atributos de um "bom dentista". Esta pesquisa de coorte contou com 79 estudantes de odontologia voluntários para participar do estudo em uma universidade pública. Os dados foram coletados pela Escala de Tendência Empática (ETE) de Dökmen e Escala de Habilidades Empáticas (EHE). Neste estudo, desde o dia da orientação até o final do terceiro ano, os escores médios do ETE dos alunos de odontologia (respectivamente 69,59 e 68,34) e os escores médios do EHE (respectivamente 142,53 e 140,00) apresentaram declínio. No final da terceira série, iniciar os rodízios clínicos e assumir a responsabilidade do paciente pode levar à diminuição dos níveis de empatia dos alunos. Os achados do nosso estudo mostram que os alunos do sexo feminino apresentam maior tendência à empatia e pontuações de habilidades em comparação aos alunos do sexo masculino (p<0,001). No ranking dos atributos necessários para ser um bom dentista, a empatia foi classificada em 4º lugar pelos alunos do primeiro ano, enquanto os alunos do último año colocaram a empatia em segundo lugar (p<0,05). De acordo com esses resultados, é possível afirmar que os candidatos a dentista perceberam que ser empático é necessário para ser um bom dentista.

ABSTRACT

El presente ensayo explora diversas perspectivas y preocupaciones relacionadas con el impacto de la inteligencia artificial (IA) en la experiencia médico paciente y la educación. El ensayo combina reflexiones narrativas y análisis crítico del problema, utilizando como recurso la novela "Fahrenheit 451" de Ray Bradbury. El autor plantea que la IA, representada, entre otros desarrollos, por los modelos de lenguaje de gran tamaño (Large Language Models ­ LLMs) como ChatGPT, tiene un impacto significativo en la medicina y la educación. A partir de la novela descrita propone preguntas fundamentales en relación con los atributos que constituyen la experiencia médicopaciente, la práctica profesional y, en general, la experiencia humana. Se analizan algunas diferencias entre los modos de razonamiento de seres humanos y sistemas algorítmicos, y se insiste en la importancia de preservar los atributos humanos en la interacción con la inteligencia artificial, como el rol de las emociones y la reflexión crítica. El artículo afirma la importancia de promover prácticas educativas fundadas en la deliberación sobre valores, el pensamiento crítico y la pedagogía sentimental, como alternativas a una relación automática con la tecnología, como expresión de una pérdida de sentido y significado: el nihilismo automático


This essay explores various perspectives and concerns related to the impact of artificial intelligence (AI) on the doctor-patient relationship and education. The essay combines narrative reflections and critical analysis of the issue, using Ray Bradbury's novel "Fahrenheit 451" as a resource. The author argues that AI, represented by developments such as Large Language Models (LLMs) like ChatGPT, has a significant impact on medicine and education. Drawing from the described novel, fundamental questions are posed regarding the attributes that constitute the doctor patient experience, professional practice, and the overall human experience. Some differences between human reasoning and algorithmic systems are analyzed, emphasizing the importance of preserving human attributes in interactions with artificial intelligence, such as the role of emotions and critical reflection. The article asserts the importance of promoting educational practices grounded in deliberation on values, critical thinking, and sentimental pedagogy as alternatives relationship with technology, as an expression of a loss of meaning and significance: automatic nihilism.

Abstract Objective Training a competent physician requires to direct the resident profile of graduate students for practice activities. We sought to identify the doctor-patient relationship orientation and the self-assessment of the core competencies, which they pointed out needed to be developed. Methods All 56 orthopedic residents admitted between 2016 and 2019 participated in the present prospective observational study. The Patient Practitioner Orientation Scale (PPOS) and a self-assessment questionnaire were answered at the beginning and end of the first year of residency (R1) in Orthopedics and Traumatology. We calculated mean and standard deviation for PPOS items and scores and analyzed them through the paired t-test. Self-Assessment Questionnaire answer options were "yes" or "I need to improve it" and skills were classified in decreasing order of the frequency of "I need to improve it" responses with description of absolute number and percentage. We compared frequencies using Fisher Test. P-values < 0.05 were considered statistically significant. GraphPad Prism 8.4.3 (GraphPad Software, San Diego, CA, USA) and Microsoft Excel (Microsoft Corporation, Redmond, WA, USA) were used for statistical analysis. Results In the period between the beginning and the end of R1, the total PPOS mean score significantly decreased from 4.63 to 4.50 (p= 0.024), more biomedical-focused. Around one-third of the residents identified competencies of patient care, practice-based learning and improvement, and interpersonal and communication skills as needed to improve. Conclusions The PPOS and self-assessment activities could promote reflection practices and are possible tools for learner-centered competency assessment. Biomedical guidance tends to prevail as the training of physicians progresses, and periodic self-assessments can be worked on to build a growth mindset.


Resumo Objetivo A formação de um médico competente requer direcionar o perfil de pós-graduação residente para atividades práticas. Buscou-se identificar a orientação de relacionamento médico-paciente e a autoavaliação das competências fundamentais que eles apontaram que precisavam ser desenvolvidas. Métodos Todos os 56 residentes em ortopedia admitidos entre 2016 e 2019 participaram do presente estudo observacional prospectivo. A Escala de Orientação Médico-Paciente (Patient Practitioner Orientation Scale [PPOS, na sigla em inglês]) e um questionário de autoavaliação foram respondidos no início e no final do primeiro ano de residência (R1) em Ortopedia e Traumatologia. Calculamos o desvio médio e padrão para itens e pontuações de PPOS e os analisamos através do teste t emparelhado. As opções de resposta do Questionário de Autoavaliação foram "sim" ou "preciso melhorar" e as habilidades foram classificadas na ordem decrescente da frequência das respostas "preciso melhorar" com descrição de número absoluto e percentual. Comparamos frequências usando o teste de Fisher. Consideramos significativos valores-p < 0,05. Os programas GraphPad Prism 8.4.3 (GraphPad Software, San Diego, CA, EUA) e Microsoft Excel (Microsoft Corporation, Redmond, WA, EUA) foram utilizados para análise estatística. Resultados No período entre o início e o final do R1, a média total de PPOS diminuiu significativamente, de 4,63 para 4,50 (p= 0,024), mais focada em biomédica. Cerca de um terço dos residentes identificou competências do cuidado ao paciente, aprendizagem e melhoria baseadas na prática e habilidades interpessoais e de comunicação, como necessitando melhorar. Conclusões As atividades de PPOS e autoavaliação podem promover práticas de reflexão e são possíveis ferramentas para avaliação de competência centrada no aluno. A orientação biomédica tende a prevalecer à medida que a formação dos médicos progride e as autoavaliações periódicas podem ser trabalhadas para construir uma mentalidade de crescimento.

Introducción. La evaluación del riesgo es fundamental en el éxito quirúrgico. Las perspectivas de los actores involucrados en el proceso de atención deben alinearse con el fin de planear, comunicar y ejecutar adecuadamente las intervenciones necesarias. El objetivo de este escrito fue analizar el riesgo quirúrgico, considerando los posibles factores relacionados con su estimación, comunicación y comprensión en la práctica clínica. Métodos. Análisis crítico y reflexivo de la información disponible, contrastado con la práctica usual. Se hace un ejercicio con situaciones clínicas habituales. Resultados. La complejidad de los pacientes, los recursos, el contexto y la naturaleza de las intervenciones, demandan una aproximación del riesgo implícito de una cirugía con instrumentos evaluativos confiables y reproducibles. Las percepciones por los médicos, pacientes y administradores en salud acerca del impacto de una cirugía difieren según sus intereses. La transmisión de los resultados es un reto ante la asimetría en el conocimiento, la complejidad de los procedimientos y la incertidumbre en los resultados. La comunicación efectiva, como una competencia, se hace altamente pertinente en la actividad del médico. Facilitar la comprensión del mensaje exige una práctica continua y un proceso de mejoramiento adaptado al contexto de la atención en salud. Conclusiones. El riesgo quirúrgico exige una metodología clara y fiable en su evaluación, comunicación y comprensión entre los actores del sistema de salud. Su presencia está asociada a la actividad profesional de los médicos y requiere competencias que permitan un abordaje no lineal del tema. Es una actividad profesional con el fin de mejorar los desenlaces en salud y la calidad de las intervenciones


Introduction. Evaluation of surgical risk is fundamental in surgical success. The perspectives of the actors involved in the care process must be aligned to adequately plan, communicate, and execute the necessary interventions. The aim of this article is to analyze surgical risk, considering possible factors related to its estimation, communication and understanding in clinical practice. Methods. Critical and reflective analysis of the available information, contrasted with the usual practice. An exercise is done with usual clinical situations. Discussion. Complexity of the patients, resources, context, and nature of the interventions demand an approximation of the implicit risk of surgery with reliable and reproducible evaluation instruments. Perceptions by physicians, patients, and health administrators about the impact of surgery differ according to their interests. The transmission of results is a challenge in the face of asymmetry in knowledge, complexity of procedures and uncertainty in results. Effective communication as a competence becomes highly relevant in the physician's activity. Facilitating the understanding of the message requires continuous practice and an improvement process adapted to the context of health care. Conclusions. Surgical risk requires a clear and reliable methodology in its evaluation, communication and understanding among the actors of the health system. Their presence is associated with the professional activity of doctors and requires skills that allow a non-linear approach to the subject. It is a professional activity that must be empowered to improve health outcomes and the quality of interventions.

ABSTRACT

Introducción: la depresión es un trastorno cada vez más prevalente alrededor del mundo. Los médicos generales son los profesionales de la salud más consultados por pacientes deprimidos. Más del 70% de los pacientes con depresión son vistos por médicos generales y no por especialistas en Psiquiatría. Según estudios realizados en Buenos Aires, más del 25% de los pacientes internados en Servicios de Clínica Médica en hospitales generales presenta depresión. Estos pacientes suelen ser atendidos y seguidos por médicos en formación, sean residentes o concurrentes de Clínica Médica. El objetivo del trabajo fue analizar el conocimiento sobre los inhibidores selectivos de la recaptura de serotonina (ISRS) que tienen los médicos residentes y concurrentes de Clínica Médica de 5 hospitales de la Ciudad Autónoma de Buenos Aires (CABA) y describir el tratamiento de un paciente depresivo por ellos. Material y métodos: se realizó un estudio descriptivo de corte transversal con un muestreo de tipo no probabilístico. Se utilizó como instrumento de medición un cuestionario semiestructurado organizado en dos secciones, una de datos demográficos que permiten caracterizar la muestra. La otra, de 15 ítems, explora los conocimientos sobre los ISRS y el tratamiento de la depresión. Dicho cuestionario fue revisado por 4 expertos. El instrumento es anónimo. Se aplicó a 59 médicos en formación en Clínica Médica, residentes y concurrentes, de 5 hospitales de la CABA, que participaron de forma voluntaria, durante el período agosto-septiembre de 2022. Resultados: la mayoría de los médicos en formación en Clínica Médica no tratan cuadros depresivos y, ante un paciente deprimido, solicitan la evaluación por un especialista en Salud Mental. Solo un 6,8% lo medica con un antidepresivo. Más del 75% de la muestra refiere recordar los conocimientos que tiene sobre de los ISRS de la cursada de Farmacología y un 13,6 de la cursada de Psiquiatría en la Facultad de Medicina. Conclusión: se observa un conocimiento deficitario sobre los ISRS en médicos residentes y concurrentes de Clínica Médica. Se considera necesario reforzar la formación sobre depresión y manejo de antidepresivos durante la residencia/concurrencia de Clínica Médica. (AU)


Introduction: depression is an increasingly common disorder around the world. General practitioners are the most frequently consulted health professionals by depressed patients. More than 70% of all depressed patients receive treatment by general practitioners and not by psychiatric specialists. According to studies conducted in Buenos Aires, more than 25% of all patients admitted to the Clinical Services in public hospitals present depression. These patients are usually under the care and follow-up of clinical trainee physicians, residents, or interns.This study aimed to analyze the knowledge about selective serotonin reuptake inhibitors (SSRIs) of clinical trainee residents and interns in five hospitals in the Ciudad Autónoma de Buenos Aires (CABA) and to describe their treatment of a depressive patient. Material and methods: we conducted a descriptive cross-sectional study with a non-probabilistic sampling. We used a semi-structured questionnaire arranged into two sections as a measuring tool. One, with demographic data to describe the sample. The other, with 15 items, explores respondents' knowledge of SSRIs and the treatment of depression. Four experts reviewed the questionnaire, which was anonymous. We applied it to 59 clinical medical trainees, residents, and interns from five CABA hospitals who volunteered to participate during August-September 2022. Results: most clinical trainees do not treat depressive conditions and, when confronted with a depressed patient, request an assessment by a Mental Health specialist. Only 6.8% medicate the patient with an antidepressant. More than 75% of the sample reported remembering their knowledge of SSRIs from the Pharmacology course and 13.6% from the Psychiatry course at the School of Medicine. Conclusion: there is a deficient knowledge about SSRIs in trainee residents and interns of Clínica Médica. We believe it is necessary to reinforce training on depression and management of antidepressants during residency/internship practice in Clínica Médica. (AU)

SUMMARY: This study investigates the relationship between the second and fourth finger ratio (2D:4D), physicians' propensity to choose an internal or surgical branch, and sex differences. On a voluntary basis, 177 physicians working in Elazig, 122 men and 55 women were enrolled in the study. Their hands were measured for 2D and 4D lengths, and the 2D:4D ratio was computed. In female doctors, the left hand's 2D:4D ratio is 1.01, compared to the right hand's 1.00. Male doctors' right 2D:4D ratio is 0.99, while their left 2D:4D ratio is 1.00. Male physicians' 2D:4D ratios were different from those of men in the general population, whereas female physicians' 2D:4D ratios were comparable to those of women in the general population. As a result, this study was the first to examine the relationship between the ratio of the second and fourth fingers (2D:4D), physicians' tendency to choose an internal medicine or surgical branch, and sex differences. While the 2D:4D ratio was higher than 0.98 in all physicians, it was low in women who disliked their profession and branch. Since there aren't many studies on this subject, data from in-depth studies that will be conducted in the future will help physicians who choose internal medicine and surgery make more informed decisions.


Este estudio investiga la relación entre la proporción de los dedos segundo y cuarto (2D:4D), la propensión de los médicos a elegir una rama interna o quirúrgica y las diferencias de género. De forma voluntaria, se inscribieron en el estudio 177 médicos que trabajaban en Elazig, 122 hombres y 55 mujeres. Sus manos se midieron en longitudes 2D y 4D, y se calculó la relación 2D:4D. En las médicos mujeres, la relación 2D:4D de la mano izquierda es 1,01, en comparación con 1,00 de la mano derecha. La relación 2D:4D derecha de los médicos hombres fue 0,99, mientras que la relación 2D:4D izquierda fye 1,00. Las proporciones 2D:4D de los médicos hombres fueron diferentes de las de los hombres en la población general, mientras que las proporciones 2D:4D de las mujeres médicas fueron comparables a las de las mujeres en la población general. Como resultado, este estudio fue el primero en examinar la relación entre la proporción del segundo y cuarto dedo (2D:4D), la tendencia de los médicos a elegir una rama de medicina interna o quirúrgica y las diferencias de sexo. Comunicar más notícias envolve, além de conhecimentos, complexidades afetivas e éticas. Sensibilizar médicos utilizando um vídeo participativo sobre a comunicação de más notícias em Unidade de Terapia Intensiva Neonatal, fundamentado no SPIKES e na antropologia visual. Qualitativa, descritiva exploratória, em três etapas: 1) Diagnóstico-participação de médicos e mães; 2) Desenvolvimento-produção do vídeo; e 3) Avaliação-percepção de médicos sobre o vídeo. Foi utilizada análise de conteúdo de Bardin.A Etapa 1 indicou: ambiência inadequada, pouca preocupação com as mães, pouco suporte emocional, inadequado convite para o diálogo, linguagem e comunicação inadequadas, pouca empatia e necessidade de melhorar a humanização. Sobre o vídeo baseado na Etapa 1, a percepção dos médicos indicou: aumento do seu grau de responsabilidade, reflexão sobre suas práticas profissionais e estímulo ao aprendizado teórico e à empatia nas relações profissionais na Unidade de Terapia Intensiva. Há dificuldades no processo de comunicação de más notícias, e a utilização de vídeo educativo participativo representa importante estratégia de incentivo à humanização da assistência em saúde.


Delivering bad news involves not only knowledge, but also affective and ethical difficulties.To raise the awareness of physicians using a participatory video about delivering bad news in an Intensive Care Therapy, based on SPIKES and on visual anthropology. Qualitative, descriptive, and exploratory study in three stages: 1) Diagnosis - participation of physicians and mothers; 2) Development - video production; 3) Evaluation - perception from physicians about the video. Bardin's content analysis was used;Stage 1 showed: inadequate environment, little concern about the mothers, little emotional support, inadequate openness for dialog, inadequate language and communication, little empathy, and need to improve humanization. About the video based on Stage 1, the perception of physicians indicated: increased degree of responsibility, reflections on their professional practices, and encouragement to theoretical knowledge and to empathy in professional relations in the Intensive Care Unit. There are issues in the process of delivering bad news, and the use of educational participatory videos is an important strategy to humanize health care.

en la actividad asistencial desarrollada por los profesionales de la salud, el consentimiento informado (CI) instrumentaliza la aquiescencia otorgada por el paciente respecto al procedimiento y/o intervención quirúrgica. Existe una regulación dispersa en el ordenamiento legal peruano en relación con el CI. En el ámbito del sistema interamericano se resuelven precedentes importantes que establecen el contenido mínimo que se debe informar al paciente previo al consentimiento. Se evalúo el cumplimiento de 22 elementos necesarios y nueve deseables en los formatos de CI, en atención al ordenamiento normativo vigente y a lo previsto a nivel supranacional por la Corte Interamericana de Derechos Humanos (Corte IDH). Dichas categorías fueron validadas mediante el juicio de expertos abogados y profesionales de la salud. La población objetivo estuvo conformada por 456 formatos de consentimiento aplicados en los establecimientos de salud públicos de Lima Metropolitana, durante los años 2019-2021. En todos los formatos se cumplen los elementos necesarios asociados con datos del paciente, el profesional de la salud y el nombre del procedimiento o intervención quirúrgica. No hay elemento deseable que se cumpla en la totalidad de estos formatos. En ese sentido, se advierten brechas en el contenido de los esos formatos en los establecimientos de salud peruanos, por lo que se debe reivindicar el aspecto informativo para lograr actos de consentimiento plenamente eficaces.


in the healthcare activity carried out by health professionals, informed consent (IC) instrumentalizes the acquiescence of the patient concerning the procedure and/or surgical intervention. There is a scattered regulation in the Peruvian legal system about IC. In the Inter-American system, essential precedents establish the minimum content the patient should be informed of before consent. Compliance with 22 necessary elements and nine desirable elements in the IC forms was evaluated under the current legal system and the supranational provisions of the Inter-American Court of Human Rights (IACHR Court). These categories were validated through the judgment of expert lawyers and health professionals. The target population consisted of 456 consent forms applied in public health facilities in Metropolitan Lima from 2019-2021. In all the formats, the necessary elements associated with patient data, the health professional, and the name of the procedure or surgical intervention are fulfilled. There is no desirable element that is fulfilled in all of these formats. In this sense, there are gaps in the content of these forms in Peruvian health facilities, so the informative aspect should be vindicated to achieve fully effective acts of consent.

Essas categorias foram validadas mediante o julgamento de especialistas advogados e profissionais da saúde. A população-alvo esteve conformada por 456 formulários de consentimento aplicados nos estabelecimentos de saúde públicos de Lima Metropolitana, durante 2019-2021. Em todos os formulários, são cumpridos os elementos necessários associados com dados do paciente, o profissional da saúde e o nome do procedimento ou intervenção cirúrgica. Não há elemento desejável que se cumpra na totalidade desses formulários. Nesse sentido, são apresentadas lacunas no conteúdo desses formulários nos estabelecimentos de saúde peruanos, por isso o aspecto informacional deve ser reivindicado para conseguir atos de consentimento plenamente eficazes.

ABSTRACT

Resumo O objetivo deste estudo foi sintetizar a literatura sobre as características de pacientes, médicos e relações médico-paciente consideradas "difíceis". Foi realizada uma revisão integrativa de estudos primários publicados entre janeiro de 2016 e setembro de 2021, em português, inglês e espanhol. De 3.414 artigos identificados nas bases Embase, PubMed, Scopus e Lilacs, 19 foram analisados qualitativamente. Sete estudos foram efetuados na Europa, oito na América do Norte, dois na América do Sul e dois na China, totalizando 1.694 pacientes, 1.903 médicos assistentes, 101 residentes e 160 acadêmicos de medicina. Médicos e/ou acadêmicos de medicina consideravam difíceis os pacientes com: condições clínicas como doenças psicossomáticas e crônicas; sintomas e queixas como dor; emoções fortes; problemas na obtenção e no compartilhamento de informações, nas tomadas de decisão, na adesão ao plano terapêutico e no seu autocuidado; e certas características sociodemográficas e de vulnerabilidade. Pacientes ou seus familiares consideravam difíceis os médicos que não os escutavam, eram preconceituosos e pareciam não se importar com seus filhos, entre outros aspectos. Sugere-se o ensino de comunicação médica e intervenções sistêmicas para melhorar as relações médico-paciente.


Abstract The scope of this study was to assess the literature on the characteristics of patients, physicians, and physician-patient relationships considered 'problematic.' An integrative review of primary studies published between January 1, 2016, and September 30, 2021, in Portuguese, English and Spanish was conducted, Of the 3,414 papers identified in the PubMed, Embase, Scopus and Lilacs databases, 19 were selected for qualitative analysis. Seven studies were carried out in Europe, eight in North America, two in South America and two in China, totaling 1,694 patients, 1,903 assistant physicians, 101 residents and 160 medical academics. Physicians and academics considered the following to be problematic: patients with clinical conditions such as psychosomatic and chronic illnesses; symptoms and complaints such as pain; powerful emotions; problems in obtaining and sharing information, in decision-making, in the adherence to the therapeutic plan and in their self-care; and some sociodemographic and vulnerability characteristics. Among other aspects, patients, or their relatives, considered physicians to be problematic when they did not listen to them or appear to care about their children, Teaching medical communication and systemic interventions are recommended to improve physician-patient relationships.

Introducción: Los procesos establecidos para el procesamiento, análisis y resultado final de las muestras de PCR, así como el control de los suministros médicos, aún se realizan de forma manual, trayendo consigo la introducción del error humano y un elevado tiempo en la entrega de los resultados. Objetivo: Presentar un sistema que automatiza los procesos de análisis, procesamiento y resultado final de las muestras de PCR, además del control de los suministros médicos en los laboratorios y centros de toma de muestras. Métodos: Con la utilización de las tecnologías para el desarrollo de programas y las herramientas de código abierto, se obtuvo un sistema automatizado para analizar y procesar las muestras de PCR. Resultados: Se implementó un sistema que automatiza el procesamiento, análisis y resultado final de las muestras de PCR, control de los suministros médicos en los laboratorios y centros de toma de muestras. Conclusiones: El sistema automatizado elimina los procesos manuales de análisis, procesamiento y resultado final de las muestras de PCR, control de los suministros médicos en los laboratorios, centros de toma de muestras y logra mayor rapidez en la respuesta a los pacientes.


Introduction: The processes established for the processing, analysis and final result of the PCR samples, as well as the control of the medical supplies to perform these, are still carried out manually, bringing with it the introduction of human error; and a high time in the delivery of the results. Objective: To present a system that automates the processes of analysis, processing and final result of PCR samples, in addition to the control of medical supplies in laboratories and sampling centers. Methods: With the use of technologies for the development of programs and open-source tools, an automated system was obtained to analyze and process the PCR samples. Results: A system was implemented that automates the processing, analysis and final result of PCR samples, control of medical supplies in laboratories and sampling centers. Conclusions: The automated system eliminates the manual processes of analysis, processing and final result of PCR samples, control of medical supplies in laboratories, sample collection centers and achieves faster response to patients.

Resumen La empatía, la comunicación efectiva y la asertividad en la práctica médica actual representan habilidades y herramientas necesarias y vigentes en un mundo de grandes avances y realidades tecnológicas que no superan, en nuestra perspectiva, lo indispensable para mantener y fortalecer la relación del profesional de la salud con el paciente, específicamente la relación médico-paciente. Es conveniente identificar y reconocer el hecho de que estas relaciones interpersonales deben ser modificadas mediante el reconocimiento de su bidireccionalidad y deben centrarse en un carácter educativo, de retroalimentación y atención mutua, con una mejora continua de la regla de las "15 C": comunicación, cercanía, comprensión, compasión, confianza, capacidad, consistencia, certificación, creatividad, cooperación/coordinación, compromiso bidireccional y conexión, con las resultantes calidad y calidez.


Abstract Empathy, effective communication and assertiveness in current medical practice represent skills and tools necessary and current, in a world of great advances and technological realities that do not surpass, in our perspective, the indispensable use of such tools to maintain and strengthen the relationship of the health professional with the patient and specifically of the doctor-patient relationship. It is convenient to identify and recognize the fact that these interpersonal relationships must be modified by recognizing their bidirectionality and that they should focus on an educational, feedback and mutual attention, with a continuous improvement of the "15 C" rule: communication, closeness, understanding, compassion, confidence, capacity, consistency, certification, creativity, cooperation/coordination and bidirectional commitment and connection, with the resulting quality and warmth.

Introdução: Este artigo explora o tema da empatia na relação médico-paciente. Objetivo: Contribuir para a habilidade de comunicação clínica por meio da estratégia de empatia proposta pela comunicação não violenta (CNV). Métodos: Estudo do principal livro de Marshall Rosenberg, Nonviolent Communication: A language of life. Subsequentemente, foi feita a análise de vários vídeos no YouTube, tanto de entrevistas como de oficinas com o próprio Rosenberg. O total de 15 horas e 8 minutos de material audiovisual foi analisado. Resultados: O conteúdo selecionado está organizado em três seções: (1) Princípios da CNV; (2) Empatia; e (3) Aplicação da empatia na prática clínica. A CNV contribui para o tema da empatia na comunicação clínica ao propor um modelo de conexão empática por meio do reconhecimento de sentimentos e necessidades de cada pessoa. Esse modelo está organizado em quatro etapas: (a) observação sem julgamento; (b) conexão com os próprios sentimentos; (c) necessidades não satisfeitas; e (d) solicitações e demandas da pessoa. Trata-se de uma síntese, não somente de comunicação, mas de uma intencionalidade e do uso consciente de uma linguagem a serviço da vida, naquilo que está vivo nas pessoas, a cada momento. Conclusões: A empatia continua sendo um tema relevante na comunicação clínica. Por se tratar de um assunto complexo, este estudo buscou ferramentas para facilitar sua aplicação prática. A CNV pode contribuir para o fortalecimento da pesquisa e o exercício da empatia na comunicação clínica ao preencher possíveis lacunas sobre o tema.


Introduction: This article explores the issue of empathy in the doctor-patient relationship. Objective: To contribute to clinical communication skills through the empathy strategy proposed by nonviolent communication (NVC). Methods: Study of Marshall Rosenberg's main book, "Nonviolent Communication: A language of life". Subsequently, several videos on YouTube were analyzed, both interviews and workshops with Rosenberg himself. A total of fifteen hours and eight minutes of audio-visual material was analyzed. Results: The selected content was organized into three sections: (1) NVC principles; (2) Empathy; and (3) Application of empathy in clinical practice. NVC contributes to the theme of empathy in clinical communication by proposing a model of empathic connection through the recognition of each person's feelings and needs. This model was organized into four stages: (a) observation without judgment; (b) connection with one's feelings; (c) unmet needs; and (d) the person's requests and demands. It is a synthesis, not only of communication, but also of intentionality and the conscious use of a language in the service of life, in what is alive in people at every moment. Conclusions: Empathy is a relevant topic in clinical communication. Because it is a complex subject, this study sought tools to facilitate its practical application. NVC can contribute to the strengthening of research and the exercise of empathy in clinical communication by filling possible gaps on the subject.


Introducción: Este artículo explora el tema de la empatía en la relación médico-paciente. Objetivo: Contribuir a las habilidades de comunicación clínica a través de la estrategia de empatía propuesta por la comunicación no violenta (CNV). Métodos: Estudio del libro principal de Marshall Rosenberg, "Nonviolent Communication: A language of life". Posteriormente, se analizaron varios videos en YouTube, tanto entrevistas como talleres con el propio Rosenberg. Se analizaron un total de quince horas y ocho minutos de material audiovisual. Resultados: El contenido seleccionado se organiza en tres secciones: (1) Principios de la CNV; (2) Empatía; y (3) Aplicación de la empatía en la práctica clínica. La CNV contribuye al tema de la empatía en la comunicación clínica al proponer un modelo de conexión empática a través del reconocimiento de los sentimientos y necesidades de cada persona. Este modelo se organiza en cuatro etapas: (a) observación sin juicio; (b) conexión con los propios sentimientos; (c) necesidades insatisfechas; y (d) las solicitudes y demandas de la persona. Es una síntesis, no solo de comunicación, sino también de intencionalidad y uso consciente de un lenguaje al servicio de la vida, en lo que está vivo en las personas, en cada momento. Conclusiones: La empatía sigue siendo un contenido relevante en la comunicación clínica. Por tratarse de un tema complejo, este estudio buscó herramientas para facilitar su aplicación práctica. La CNV puede contribuir al fortalecimiento de la investigación y al ejercicio de la empatía en la comunicación clínica llenando posibles vacíos sobre el tema.

ABSTRACT

A violência contra profissionais de saúde é um problema mundial de saúde pública. Os episódios de violência têm impacto significativo no bem-estar físico e psicológico dos profissionais, com repercussão nos serviços de saúde prestados aos doentes. A avaliação dos fatores de risco para a violência e dos motivos que levam à subnotificação permite esclarecer e alertar os profissionais de saúde para a violência contra eles. O desenvolvimento e a implementação de programas contra a violência no local de trabalho constituem ainda uma das estratégias para a redução e evicção da violência. Este artigo teve como objetivo abordar a temática da violência contra os profissionais de saúde, nomeadamente os médicos de família, para discutir a prevalência dos episódios de violência, os seus diferentes tipos, principais fatores de risco, impacto na saúde física e psicológica dos profissionais de saúde, impacto na prestação de cuidados e estratégias de prevenção e gestão destes.


Violence against healthcare professionals is a global public health problem. Incidents of violence have a significant impact on the physical and psychological well-being of professionals, which in turn affects the quality of care provided to patients. The assessment of risk factors for violence and the reasons behind underreporting them helps clarify and raise awareness among healthcare professionals about violence against them. The development and implementation of programs against violence in the workplace are some of the strategies aimed at reducing and eliminating violence. This article aimed to address the issue of violence against healthcare professionals, particularly Family Physicians, by discussing the prevalence of violent episodes, different types of violence, main risk factors, impact on the physical and psychological health of healthcare professionals, impact on care provision, and strategies for prevention and management.


La violencia contra los profesionales de la salud es un problema mundial de salud pública. Los episodios de violencia tienen un impacto significativo en el bienestar físico y psicológico de los profesionales, lo que repercute en los servicios de salud brindados a los pacientes. La evaluación de los factores de riesgo de violencia y las razones que llevan a su subnotificación permite aclarar y alertar a los profesionales de la salud sobre la violencia contra ellos. El desarrollo e implementación de programas contra la violencia en el lugar de trabajo son algunas de las estrategias para reducir y eliminar la violencia. Este artículo tuvo como objetivo abordar el tema de la violencia contra los profesionales de la salud, especialmente los médicos de familia, al discutir la prevalencia de los episodios violentos, los diferentes tipos de violencia, los principales factores de riesgo, el impacto en la salud física y psicológica de los profesionales de la salud, el impacto en la prestación de atención y las estrategias de prevención y manejo.

SUMMARY INTRODUCTION: We observe a growing global discussion about the practices considered "obstetric violence" against women during pregnancy and childbirth. Otherwise, the indiscriminate subjective and lay interpretation of the term "obstetric violence" can lead to a misunderstanding among medical professionals. OBJECTIVE: This study aimed to describe the obstetrician's perceptions about the term "obstetric violence" and the medical groups affected negatively by the topic. METHODS: A cross-sectional study applied to Brazilian obstetrics physicians regarding their perceptions of "obstetric violence." RESULTS: From January to April 2022, we sent about 14,000 direct mail nationwide. A total of 506 participants responded. We observed that 374 (73.9%) participants consider the term obstetric violence nocive or harmful to professional practice. Furthermore, after Poisson regression, we described that the respondents who graduated before 2000 and from a private institution were significant and independent groups for the full or partial agreement that the term is nocive for the obstetricians in Brazil. CONCLUSION: We observed that almost three in four obstetrician participants consider the term "obstetric violence" nocive or harmful to professional practice, particularly in those who graduated before 2000 and from a private institution. The findings are relevant to propose further debates and strategies to mitigate the possible harms caused to the obstetrician team by the indiscriminate use of the term obstetric violence.

Abstract Introduction Patient-physician relationship is associated with ART adherence and medical follow-up in people living with HIV (PLWH). Patient's trust in their doctor is a key component of patient-physician relationship, so adequate and reliable instruments to measure this component are important to evaluate its impact on health outcomes. Objective To evaluate the psychometric properties of a translated and adapted version of Trust in Physician Scale (TPS) in Mexican PLWH. Method A cross-sectional study was carried out in PLWH. Scale was translated to Spanish and culturally adapted. Sociodemographic and TPS data were collected online due to COVID-19 pandemic. Exploratory (EFA) and confirmatory (CFA) factor analysis were carried out in two different samples. Results Data from 215 participants was used to EFA. Five items were eliminated due to low correlation with total scale. Final Cronbach's alpha was .93. A single-factor structure explained 68.8% of the variance. CFA in a sample of 140 participants confirmed adequate fit indices (χ2[7] = 13.015 p = .072, CFI = .997, RMSEA = .057, SMRS = .0015). Discussion and conclusion The final scale was unifactorial and it is made up of six items instead of 11. It seems to be a valid and reliable scale to measure patient's trust in doctors in Mexican PLWH. Further studies are recommended to provide evidence of convergent validity to the instrument.


Resumen Introducción La relación médico-paciente está asociada a la adherencia al tratamiento antirretroviral y al seguimiento médico en las personas que viven con VIH (PVVS). La confianza de los pacientes en sus médicos es un componente clave de la relación médico-paciente, por lo que es importante disponer de instrumentos adecuados y fiables para medir este componente y evaluar su impacto en los resultados de salud. Objetivo Evaluar las propiedades psicométricas de una versión traducida y adaptada de la Trust in Physician Scale (TPS) en PVVS mexicanas. Método Se realizó un estudio transversal en adultos con VIH. La escala fue traducida al español y adaptada culturalmente. Los datos sociodemográficos y de la TPS se recogieron en línea debido a la pandemia de COVID-19. Se realizaron análisis factoriales exploratorios (AFE) y confirmatorios (AFC) en dos muestras diferentes. Resultados Se utilizaron los datos de 215 participantes para el AFE. Se eliminaron cinco ítems debido a la baja correlación con la escala total. El alfa de Cronbach final fue de .93. Una estructura unifactorial explicó el 68.8% de la varianza. El AFC en una muestra de 140 participantes confirmó la adecuación del modelo mostrando índices de ajuste adecuados (χ2[7] = 13.015 p = .072, CFI = .997, RMSEA =.057, SMRS = .0015). Discusión y conclusión La escala final fue unifactorial y se compuso de seis ítems en lugar de 11. Parece ser una escala válida y fiable para medir la confianza del paciente en los médicos en PVVS mexicanas. Se recomiendan más estudios para buscar evidencia de validez convergente del instrumento.

ABSTRACT

Objective:To investigate the association between cancer fatalism and optimistic attitudes among colorectal cancer patients, and how the above linkage is moderated by the involvement of patients′ opinions, the family members′ opinions and the physicians′ opinions in treatment decision-making.Methods:A cross-sectional survey was conducted on 64 patients diagnosed with colorectal cancer and awaiting radical colorectal cancer surgery in the Department of General Surgery of Beijing Friendship Hospital Affiliated to Capital Medical University, from January 2021 to December 2021. There were 38 males and 26 females, aged (61.2±13.0) years from 30 to 84 years. Cancer fatalism, optimism attitudes, and the involvement of patients′ opinions, the family members′ opinions and the physicians′ opinions in treatment decision-making were assessed. The patients′ cancer fatalism beliefs were measured by the " Chance" subscale of the Form C of the Multidimensional Health Locus of Control (MHLC-C), optimism attitudes were assessed by the Chinese version of the revised Life Orientation Test (CLOT-R), and the influence of patients′, family members′, and physicians′ opinions in the medical decision-making process was measured by a self-designed single factor scale. Measurement data of normal distribution were expressed as mean±standard deviation ( ± s). Measurement data of skewed distribution were expressed as M( Q1, Q3). Spearman correlation analysis and Chi-square test was used to examine the association between the main outcome variable (patients′ optimistic attitudes) and demographic and clinical characteristics. Interaction was examined by hierarchical linear regression analysis combined with simple slope tests. Results:Cancer fatalism was negatively associated with patients′ optimistic attitudes ( r=-0.35, P<0.01). Optimistic attitude of patients was significantly and negatively correlated with the influence of family members′ opinions on medical decision-making ( r=-0.25, P<0.05). There were significant positive correlations between the influence of patients′ and family members′ opinions ( r=0.50, P<0.01), family members′ and physicians′ opinions ( r=0.67, P<0.01), and physicians′ and patients′ opinions ( r=0.38, P<0.01) in medical decision making. Hierarchical linear regression analysis showed a negative association between cancer fatalism and optimism ( β=-0.32, P=0.01). This association was further moderated by the involvement of family members′ opinions ( β=-0.56, P<0.01) and the involvement of physicians′ opinions ( β=-0.36, P=0.04) in medical decision-making. Simple slope tests revealed that the negative impact of fatalistic attitudes on patients′ optimism attitudes may be potentiated when family members′ opinions have high influence on medical decision-making, while the negative impact may be buffered to some extent when physicians′ opinions have high influence on medical decision-making. Conclusions:Cancer fatalism had a negative effect on patients′ optimism. The high influence of physicians in treatment decision-making buffered the negative effect of cancer fatalism on optimism; the high influence of family members in treatment decision-making potentiated the negative effect of cancer fatalism on optimism. In the process of doctor-patient communication and shared decision-making, for patients with strong fatalistic attitudes, consideration should be given to appropriately increasing direct informational communication between physicians and patients and reducing excessive family intervention in medical decision-making, so as to enhance patients′ autonomy for treatment, promote optimism, and reduce the negative effects of cancer fatalism.

ABSTRACT

Objectives To learn the echocardiography skills of intensivists after receiving a basic critical care echocardiography training course, and investigate factors that may influence their performance. Methods We completed a web-based questionnaire that assessed the skills in ultrasound scanning techniques of intensivists who took a training course on basic critical care echocardiography held in 2019 and 2020. Mann-Whitney test was used to analyze the factors which might affect their performance on image acquisition, recognizing clinical syndrome, and measuring the diameter of inferior vena cava, left ventricular ejection fraction and left ventricular outflow tract velocity-time integral.Results We enrolled 554 physicians from 412 intensive care units across China. Among them, 185 (33.4%) reported that they had 10%-30% chance of being misled by critical care echocardiography when making therapeutic decision, and 34 (6.1%) reported that the chance was greater than 30%. Intensivists who performed echocardiography under the guidance of a mentor and finished ultrasound scanning more than 10 times per week reported significant higher scores in image acquisition, clinical syndrome recognition, and quantitative measurement of inferior vena cava diameter, left ventricular ejection fraction and left ventricular outflow tract velocity-time integral than those without mentor and performing echocardiography 10 times or less per week respectively (all P < 0.05).Conclusion The skills in diagnostic medical echocardiography of Chinese intensivists after a basic echocardiographic training course remain low, and further quality assurance training program is clearly warranted.
